Before the late 1900s, many pharmaceutical companies managed clinical trials in-house.1,2 In the early 2000s, clinical trials began to become more complex while regulatory demands became more stringent. Additionally, since there was a need for faster patient recruitment and improved data quality, clinical trials needed a more efficient process to continue to conduct studies with reduced costs and risk of errors.3 Clinical trials are critical in drug development, but often face challenges, such as  delays and difficulties with patient recruitment. The increasing complexity of trials has also created a greater need for specialized expertise. These factors have led to increased reliance on site management organizations (SMOs), which specialize in handling trial operations.4

SMOs often partner with sponsors to help manage and conduct clinical trials. SMOs have a variety of important functions such as evaluating a site’s capabilities and using networks and databases to match trial protocols with appropriate sites.3,5 These organizations are not only knowledgeable about both local regulatory requirements and international requirements.3 They often have to partner with regulatory agencies to develop standardized processes and guidelines.3 By providing specialized operational support, SMOs can help reduce the complexity and costs of clinical trials and enable more efficient drug development. 

Although SMOs play a critical role in supporting the execution of clinical trials, they also face many challenges. The regulatory environment is constantly changing, and regulations differ by country and region, making compliance difficult, especially for international studies.3  Therefore, SMOs must stay updated on evolving regulations and ensure that all trial sites adhere to regulatory standards.

In addition to navigating regulatory requirements, SMOs also have expertise in patient recruitment.3 Because patient recruitment has been a significant challenge in clinical trials, SMOs often use targeted advertising and community outreach to enhance enrollment. Many SMOs now incorporate patient-centric approaches such as patient advisory boards and patient education programs.3 They have also developed retention strategies to keep participants engaged throughout the course of studies.6

Another important area of SMO expertise is data management, which involves overseeing accurate data collection to ensure that sponsors can obtain high-quality data for analysis.7 They have robust data management systems to collect and store trial data. Data management support may include study tracking, query coordination, data workflow support, participant tracking oversight, reporting support, and operational data coordination activities throughout the study lifecycle.8 To ensure the reliability of trial data, SMOs also have to implement stringent quality assurance measures such as monitoring and auditing of trial sites.3  Quality assurance is essentially a system for ensuring a desired level of excellence.9

Partnering with SMOs allows sponsors or pharmaceutical companies to focus on core research and development activities. Sponsors who partner with SMOs have reported faster site activation timelines, higher enrollment rates, and fewer protocol deviations.10 SMOs help reduce operational fragmentation through their various key functions, and given today’s trial site landscape, SMOs now play a major role in supporting multi-center and international studies.2
